Process For Obtaining A COI

Three things to know

Certificates. Do you need a certificate of insurance for a field, a landlord, or an event host?

Email [email protected] with your club’s name, the venue or landlord’s name and address, and the dates. Name certificates come back within one business day.

Incidents. If a player is hurt or something happens at a match or practice, email UPSL at [email protected] the same day with the date, venue, who was involved, and any documents. UPSL reviews it and submits it to Golden. Report early, even if you are not sure it qualifies.

Waivers. Every participant signs a waiver before their first participation, and your club keeps it on file. Without one, that participant is not covered.
